To fix this, start by dialing *#62# to check if any of your services is being forwarded. If you one of more is forwarded, then you have your answer. Simply dial ##002# to cancel the forwarding. Then dial *#62# to be sure its done.
For the other issues like spyware; Spyware apps regularly eat up your data. If you suspect that your phone may be tapped, start keeping an eye on your data. Then go to Apps option on your phone and disable or uninstall any App that looks suspicious.
You should be fine after doing these.
